The brain itself doesn’t have pain-sensitive receptors—so it can’t actually feel pain. However, try telling that to someone experiencing a headache or migraine! The pain we associate with headaches usually comes from the tissues surrounding the brain, such as the meninges (the protective coverings), as well as the scalp, face, and outer parts of the head. These structures contain pain receptors and are often the true source of headache pain.

Another cause of headaches, and probably the most common, is the neck. Issues with the neck may refer pain to some part of the head – you might feel it behind the eyes, in the forehead or back of the head.

Migraines, on the other hand, are a completely different story. They affect 15 per cent of people, and their cause is largely unknown. Migraine symptoms vary greatly from a throbbing head, pain in the face, sensitivity to light and/or sound, vomiting, nausea and visual disturbances, to name a few. In his groundbreaking book Migraine, Dr Oliver Sacks identified almost a hundred distinct forms of migraine—showing just how complex this condition can be.

They often have multiple triggers. Common causes include musculoskeletal dysfunction in the neck, poor posture, stress or tension, hormonal changes, faulty movement patterns, and neurological sensitisation. Tight neck/shoulder muscles can aggravate or trigger attacks.

Targeted exercises, manual therapy, and lifestyle advice can all play a role in decreasing migraine triggers and improving overall function.

The practitioner will take your full history (trigger factors, onset, duration), test neck, jaw, posture, movement, muscle strength and flexibility. They may try to replicate your symptoms to pinpoint contributing structures.

It depends on severity, chronicity and adherence to home programs. Many people notice benefits within 4–6 weeks (6–10 sessions), but others may need longer or ongoing maintenance.

  • Maintaining good posture (especially when working/at screens)
  • Doing prescribed neck, shoulder or jaw mobility and strengthening exercises
